About the position

Kaiser Permanente (KP) is seeking a Licensed Practical Nurse (LPN) to join our team in Jonesboro, Georgia. In this role, you will advance the art and science of nursing through clinical and service excellence, professionalism, and leadership. As an LPN, you will provide essential support for team and provider(s) during facility encounters, manage patient traffic flow, and perform follow-up care as per provider instructions and within your scope of practice. You will also assist with patient education and document clinical information into the medical record, ensuring that all patient interactions are recorded accurately and in compliance with guidelines. Your responsibilities will include preparing patients for exams by reviewing their medical history, collecting current information, and taking appropriate vital signs. You will address POE Care Gaps and schedule appointments when necessary, verify current medications, and pend refill medications for provider signature. Additionally, you will review and update patient allergies, smoking, and tobacco history, and provide interventions ordered by practitioners, such as administering medications, changing dressings, collecting specimens, and performing suture removals and IVs. You will also prepare rooms and equipment for procedures and assist physicians as needed. In this position, you will play a crucial role in educating patients and providing them with preparation materials for diagnostic studies. You will schedule appointments for patients with lab, radiology, and follow-up appointments as ordered by the practitioner. Your ability to effectively provide preventative and ongoing health education to patients and their families will be essential. You will also be responsible for resolving patient concerns promptly and participating in emergency care as necessary, under the direction of a physician or registered nurse. Maintaining department Quality Improvement standards will be a key aspect of your role as well.

Responsibilities

  • Prepares patient for exams by reviewing history, collecting current information and appropriate vital signs, documenting in the medical record according to guidelines.
  • Addresses POE Care Gaps and schedules appointments when appropriate.
  • Verifies all current medications and pend refill medications needed for provider signature.
  • Reviews, verifies, and updates allergies, smoking, and tobacco history.
  • Provides intervention ordered by practitioner (e.g., medication, dressings, specimen collections, irrigations, suture removals, and IVs) and documents patient response.
  • Prepares room and equipment for procedures and assists physician as needed.
  • Assists with monitoring and management of patient and physician electronic messages as delegated by the RN and within scope of practice.
  • Educates patient and provides preparation materials for diagnostic studies according to guidelines.
  • Schedules appointments for patients with lab, radiology, consults, and follow-up appointments as ordered by practitioner.
  • Effectively provides preventative and ongoing health education to patients and family members.
  • Resolves patient concerns promptly.
  • Participates in emergency care, as necessary, and as directed by physician or registered nurse.
  • Maintains department Quality Improvement standards.

Requirements

  • Graduate of an accredited nursing (LPN/LVN) program.
  • Practical Nurse License (Georgia) required at hire.
  • Basic Life Support required at hire from American Heart Association.
