About the position

The Licensed Professional Mental Health Counselor - Admissions Case Manager position at the Veterans Health Administration involves providing comprehensive mental health services to veterans. The role focuses on assessing the psychosocial needs of veterans, developing treatment plans, and coordinating care with interdisciplinary teams to ensure effective delivery of health services. The counselor will utilize clinical judgment and evidence-based practices to support veterans in achieving their recovery goals while advocating for their needs within the VA and community resources.

Responsibilities

  • Assess and document behaviors or symptoms of abuse, neglect, or exploitation.
  • Evaluate the psychosocial functioning and needs of veterans, collaborating with family members and interdisciplinary teams.
  • Make decisions regarding the use of VA and non-VA services and referrals based on initial assessments.
  • Implement clinically indicated and evidence-based treatment to assist veterans in achieving identified goals.
  • Provide case management and care coordination to facilitate appropriate delivery of health care services.
  • Collaborate with interdisciplinary teams to develop discharge plans and set achievable goals for veterans.
  • Coordinate post-discharge services including VA and community-based services.
  • Attend interdisciplinary treatment team meetings and update treatment and discharge plans as necessary.
  • Act as an advocate for veterans with appropriate VA and community service providers.
  • Maintain a current network of referral resources and make referrals to other services as needed.
  • Conduct biopsychosocial assessments with cultural competence.
  • Provide professional social work services to veterans experiencing various issues, including substance use disorders.
  • Utilize crisis intervention techniques and implement safety plans for at-risk veterans.
  • Develop and facilitate psychotherapy and psycho-education groups.

Requirements

  • Must be a U.S. Citizen.
  • Proficient in written and spoken English.
  • Hold a master's or doctoral degree in Clinical Mental Health Counseling or a related field from an accredited program.
  • Must have a full, current, and unrestricted license to practice as a Licensed Professional Mental Health Counselor.
  • Demonstrated ability to engage in intake and assessment, problem identification, diagnosis, and treatment planning.
  • Knowledge of the DSM for formulating treatment goals and applying clinical interventions.
  • Ability to use a variety of counseling interventions and demonstrate sensitivity to diversity.
  • Basic computer skills for documentation and data management.

Nice-to-haves

  • Experience working with veterans or in a VA setting.
  • Advanced knowledge of crisis intervention and stabilization techniques.
  • Experience in developing and facilitating group therapy sessions.
