About the position

The Licensed Professional Mental Health Counselor (Care Coordinator) position at the Veterans Health Administration is responsible for providing mental health care coordination for eligible Veterans. This role involves conducting clinical psychosocial assessments, care management, and various forms of therapy including individual, couples, family, and group therapy. The counselor will work independently to identify high-risk patients, provide crisis intervention, and coordinate community-based services, ensuring comprehensive support for Veterans' mental health needs.

Responsibilities

  • Provide mental health care coordination for eligible Veterans in outpatient clinics.
  • Conduct clinical psychosocial assessments and develop treatment plans in collaboration with Veterans and interdisciplinary teams.
  • Perform case/care management functions and outreach to identify and screen patients for psychosocial needs.
  • Provide individual, couples, family, and group therapy, including substance use disorder specific services.
  • Identify high-risk patients and provide crisis intervention services.
  • Coordinate community-based services and referrals for additional support.
  • Develop patient education materials and assist with Veteran Peer and Family activities.
  • Consult with other staff regarding the psychosocial needs of Veterans and the impact on health care adherence.
  • Participate in administrative meetings and training activities.
  • Assume responsibilities associated with committee assignments, including Performance Improvement and Peer Review.

Requirements

  • Must be a U.S. Citizen.
  • Hold a master's or doctoral degree in Clinical Mental Health Counseling or a related field from an accredited program.
  • Possess a full, current, and unrestricted license to independently practice as a Licensed Professional Mental Health Counselor.
  • Demonstrated ability to engage in intake and assessment, problem identification, diagnosis, treatment planning, and implementation.
  • Knowledge of current DSM for treatment goal formulation and clinical interventions.
  • Ability to communicate effectively with diverse populations.

Nice-to-haves

  • Experience in crisis intervention and stabilization for patients in psychological distress.
  • Familiarity with telehealth services and software applications for clinical documentation.
  • Skills in developing and facilitating psychotherapy and psycho-education groups.
