About the position

The Licensed Psychiatric Social Worker at VNS Health provides essential client-centered behavioral health care to individuals in New York, addressing their mental health needs across various settings, including outpatient clinics and clients' homes. This role involves delivering outreach services, conducting psychosocial evaluations, and creating service plans in collaboration with an interdisciplinary team. The position is integral to ensuring comprehensive care for vulnerable populations, including children, adolescents, and aging adults, while also offering opportunities for professional growth and skill development.

Responsibilities

  • Delivers outreach services to mentally ill individuals in the community, providing in-home mental health intervention.
  • Collaborates with team members to perform psychosocial evaluations and assess mental health service needs of clients and their families.
  • Creates and executes short-term service plans for clients in collaboration with an interdisciplinary team.
  • Responds promptly to dispatch calls, traveling from the office to clients' homes within two hours.
  • Acts as a vital liaison to community agencies and steps in for the Program Coordinator as needed.
  • Ensures uninterrupted comprehensive care from counseling and care management to long-term treatment referrals and follow-up.
  • Prepares case histories and maintains case records in accordance with the Program record-keeping mechanism.
  • Encourages resistant clients to accept mental health services through interventions with clients and/or family members.
  • May provide clinical supervision for Mental Health Technicians, graduate Social Worker students, or junior staff Social Workers.

Requirements

  • Current registration to practice as a Licensed Master Social Worker in New York State.
  • Minimum of one year experience as a Social Worker in a health care setting.
  • Master's degree in social work from an accredited graduate school of Social Work.
  • Valid driver's license may be required.
  • Bilingual skills may be required.
