The Social Worker (MSW) position at Hartford HealthCare involves providing personalized care to patients in their homes, focusing on their short and long-term needs. The role requires collaboration with patients, families, and healthcare teams to address bio-psychosocial needs and implement care plans. The Social Worker will utilize their clinical skills to conduct assessments, provide counseling, and advocate for patients, ensuring they receive the necessary support and resources.

Responsibilities

  • Collaborate with the interdisciplinary team regarding patients identified for Social Work Services.
  • Complete comprehensive in-home psychosocial evaluations considering various factors such as developmental, social, and medical issues.
  • Create care plans with measurable goals based on culturally competent assessments.
  • Implement short-term interventions to address identified barriers and promote health and well-being.
  • Provide clinical treatment services including triage, crisis intervention, and therapy.
  • Utilize Motivational Interviewing and Coaching skills to assist patients and families in identifying goals.
  • Educate patients, families, and the community on disease prevention and health maintenance.
  • Advocate for patients' needs to improve access to care and service delivery.
  • Monitor and alter care plan goals and interventions as necessary.
  • Maintain timely documentation of social work services and outcomes.

Requirements

  • 2-3 years of healthcare experience, including acute and community health experience.
  • Licensure as LMSW (Licensed Master Social Worker).
  • Considerable knowledge of social, cultural, economic, and psychological issues affecting patients and families.
  • Familiarity with evidence-based assessment tools such as PHQ-9, SLUMS, and others.
  • Knowledge of community resources and eligibility requirements.
  • Proven ability to work effectively both independently and in a team environment.
  • Excellent written and verbal communication skills.
  • Strong organizational skills and attention to detail.
  • Strong computer skills, including Microsoft Office; Epic experience is a plus.
