About the position

The Licensed Social Worker Staff (MSW, LSW) in Clinical Case Management for the Emergency Department at Carle Foundation Hospital plays a crucial role in supporting patients and their families by addressing psychosocial factors that influence health. This position involves conducting assessments, providing crisis intervention, and facilitating access to healthcare resources, all while collaborating with an interdisciplinary team to ensure comprehensive patient care.

Responsibilities

  • Perform psychosocial assessments to identify patient needs and barriers to care.
  • Provide crisis intervention and supportive counseling to patients and families.
  • Educate patients and families about Advanced Directives and facilitate advanced care planning conversations.
  • Maintain knowledge of community resources and refer patients to necessary services.
  • Assist with care transitions and collaborate with community services for patient safety.
  • Document assessments and interventions in the electronic health record according to standards.
  • Advocate for patient confidentiality, informed consent, and self-determination.
  • Assess patient safety and collaborate with the care team to address safety issues.
  • Provide education to patients and families regarding available services and supports.
  • Participate in mentoring new employees and supervising social work interns as needed.
  • Facilitate interdisciplinary team meetings to review patient care plans.

Requirements

  • Master's Degree in Social Work from an accredited school of social work.
  • Licensed Social Worker (LSW) in good standing in the state of practice.
  • Strong knowledge of the social work process and healthcare system.
  • Excellent interpersonal skills and ability to work collaboratively in a team.
  • Strong verbal and written communication skills.
  • Knowledge of social determinants of health and cultural diversity.

Nice-to-haves

  • Experience in crisis intervention and supportive counseling.
  • Familiarity with advanced care planning and community resources.
