About the position

The Licensed Vocational Nurse I (LVN) plays a crucial role in providing in-office practice management support within a primary care practice, operating under a Team Based Care model. The LVN is responsible for delivering clinical expertise to ensure high-quality patient care, offering telephone advice for minor acute issues, educating patients on self-care, and assisting in the management of chronic diseases. This position involves a variety of nursing procedures and patient monitoring, contributing to the overall efficiency and effectiveness of the healthcare team.

Responsibilities

  • Serve as a resource for Medical Assistants (MA) and Providers, providing patient education on self-care and medication use.
  • Oversee the intake of patient telephone calls, providing advice on minor acute problems and referring to the appropriate healthcare provider as needed.
  • Participate in patient follow-up activities, including reporting diagnostic test results and arranging prescription refills.
  • Assist in the ongoing management of patients with stable chronic diseases, coordinating treatment plans and services with the healthcare team.
  • Triage walk-in/same-day patients, monitoring and providing necessary nursing interventions for acute medical conditions.
  • Perform nursing procedures such as injections, nebulizer treatments, and wound care as required by clinical needs.
  • Administer medications and immunizations to pediatric patients as prescribed.
  • Observe, record, and report changes in patients' conditions in a timely manner.
  • Support the Patient Assistance Program (PAP) by tracking drug applications and coordinating care for patients.
  • Assist in maintaining clinical supplies and medications inventory.
  • Serve as a training resource for clinical support staff and assist in developing training materials.
  • Participate in quality improvement projects related to patient care.

Requirements

  • High School diploma or G.E.D. and graduation from an approved licensed practical nurse/vocational nurse (LPN/LVN) program.
  • Current license as a Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN) by the State of Texas Board of Nurse Examiners.
  • Basic Life Support certification (BLS).

Nice-to-haves

  • Experience with Patient-Centered Medical Home (PCMH) principles and quality processes.
  • Experience or interest in working with homeless or underserved populations.
  • Pediatric experience.
