Licensed Vocational Nurse (LVN) - Fremont, CA

About the position

The Licensed Vocational Nurse (LVN) at Elwyn plays a crucial role in providing nursing care and support to individuals with autism, intellectual, developmental, and behavioral challenges. This position involves performing general nursing procedures, participating in interdisciplinary team efforts, and ensuring adherence to care standards. The LVN may also oversee direct support professionals and contribute to the development of individual support plans, all while working in a supportive and flexible environment.

Responsibilities

• Perform general nursing procedures as authorized within the scope of practice.
• Participate as a member of the interdisciplinary team and contribute to the development of the Individual Support Plan.
• Function as lead staff on the shift and oversee Direct Support Professionals (DSPs) and/or Certified Nursing Assistants (CNAs).
• Provide active programming at all times.
• Ensure adherence to standards, procedures, and schedules established by the supervisor.
• Report any suspicion of or knowledge of suspected abuse in accordance with the law and program policies.

Requirements

• Valid California driver's license and acceptable driving history.
• High school diploma or GED.
• Possess and maintain a valid, active license from the State of California to practice as a Licensed Vocational Nurse or Licensed Psychiatric Technician.
• Minimum 1 year experience in the Human Services field preferred.
