About the position

The Licensed Vocational Nurse (LVN) position at Focused Post Acute Care Partners involves providing nursing care to residents as prescribed by physicians and in accordance with nursing standards and regulations. The role includes leading and directing both licensed and non-professional staff in delivering direct resident care and support functions, ensuring the safety and well-being of patients.

Responsibilities

• Provide nursing care to residents as prescribed by the physician.
• Lead or direct licensed and non-professional staff in the delivery of direct resident care.
• Plan, organize, and direct nursing functions for patients in the unit.
• Make nursing assignments appropriate to the skill level of employees.
• Maintain acceptable standards of patient care and identify problems for resolution.
• Create a working climate that promotes growth and job satisfaction for personnel.
• Accurately implement physicians' orders and maintain confidentiality.
• Support and enforce infection-control policies and procedures.
• Obtain and document comprehensive nursing history from patients and/or significant others.
• Perform basic assessments of patients and record vital signs, noting changes.
• Develop care plans based on assessment information before the end of duty time.
• Communicate effectively with patients, families, and healthcare team members.
• Respond appropriately in urgent and emergency situations.

Requirements

• Must have a current LVN license.
• CPR certification required.
• Ability to understand, remember, and carry out verbal or written instructions in English.
• Ability to complete assignments in a timely manner.
• Ability to be flexible and positive with intermittent interruptions.
