About the position

The City of Saint Paul is seeking Life Coach Case Managers to play a critical role in reducing gun violence and providing ongoing case management support to Project PEACE programming. These positions involve working closely with young adults identified as being at high risk of gun violence or those who have been victims of such violence, along with their families. Life Coaches will establish trusting relationships with clients, helping them mitigate risk factors and connect to necessary services and supports.

Responsibilities

• Provide in-person client connection with assigned clients to assess their needs in education, employment, housing, food, social/emotional, and behavioral areas.
• Assess challenges and barriers related to justice involvement and educational or vocational needs.
• Apply for immediate and emergency services as necessary for family preservation.
• Develop ongoing service plans for families and individuals, including community referrals and necessary documentation.
• Engage clients in ongoing treatment processes through individual and group counseling, establishing goals and assessing progress.
• Coordinate care and appointments for clients related to physical, mental, and behavioral health, as well as justice involvement.

Requirements

• Minimum of four years of individual and family crisis counseling/life coaching experience, preferably with a high-risk client population.
• Bachelor's degree in a related field or equivalent experience.
• Training and experience in assessing the vocational and educational needs of clients, or the ability to attend training within 6-9 months of employment.
