The Social Worker plays a crucial role in ensuring that the emotional and social needs of patients and their families are met and maintained in accordance with facility guidelines and federal/state regulations. This position requires sensitivity to the patient's social, ethnic, religious, and cultural needs, ensuring that care is tailored to the individual circumstances of each patient. The Medical Social Worker collaborates with other healthcare providers to assess and plan interventions that address any issues impacting the patient's recovery. This includes working closely with medical teams to develop comprehensive care plans that consider the patient's social circumstances, support systems, and any barriers to recovery. The role is vital in facilitating communication between patients, families, and healthcare providers, ensuring that all parties are informed and involved in the care process. The Social Worker also provides counseling and support to patients and families, helping them navigate the complexities of the healthcare system and access necessary resources. In addition to direct patient care, the Social Worker is responsible for maintaining accurate documentation of patient interactions and care plans, ensuring compliance with all relevant regulations and standards. This position requires a proactive approach to problem-solving and a commitment to providing high-quality care to all patients.

Responsibilities

Requirements

  • High School Diploma/GED
  • Master's Degree
  • Arizona Licensed Medical Social Worker in good standing
  • 3 years experience in social work
  • Thorough working knowledge of Medicare, Medicaid, and Managed Care
  • Familiarity with federal, state, and accreditation standards of Social Work in an acute care setting.

Nice-to-haves

  • 1-3 years experience in a similar role
