This LMSW position focuses on providing social services to seniors in the Omaha area. The role involves collaborating with primary care teams to assess the needs of seniors and implementing support packages to address socioeconomic and medical barriers. Responsibilities

• Develop relationships and collaborate with primary care teams to conduct social services assessments to determine the appropriate needs for each member.
• Plan, coordinate, manage, and implement support packages to help members deal with socioeconomic and medical barriers.
• Navigate managed care plans for community services and programs.

Requirements

• Fully licensed LMSW in Nebraska.
• Master's degree in Social Work.
• 1+ years' experience in clinical social work.
• Experience working with patients in need of social services.
• Experience with assessments and planning for social services.
• Working knowledge of different diagnoses of common health conditions.
• Proficient PC skills.
• Fluency in Spanish or other language spoken by the communities served.
