About the position
The Long-Term Care Social Worker at Bartlett Regional Hospital is responsible for providing social work services to residents within the Wildflower Court facility. This role involves creating individualized treatment plans in collaboration with the Director of Post-Acute Care, the Director of Nursing, and family members or guardians. The Social Worker operates independently with limited supervision, managing both routine and emergency care while adhering to established policies and ethical guidelines.
Responsibilities
- Provide social work services to residents within the Wildflower Court facility.
- Develop and implement individualized treatment plans for residents.
- Collaborate with the Director of Post-Acute Care and Director of Nursing.
- Work with family members and guardians to ensure effective care.
- Stay updated on policies, procedures, ACMA guidelines, and NASW code of ethics.
- Manage routine and emergency care as needed.
Requirements
- Graduate of an accredited social work program with a bachelor's degree.
- Alaska Social Work License at time of appointment or within 90 days of hire if licensed in another state.
- One year of experience working as a social worker in health care or long-term care.
Nice-to-haves
- Master's degree in social work.
- Valid AK Driver's License if assigned to perform patient transport duties.
Benefits
- $2 an hour casual differential in addition to hourly base pay.
