About the position

The Long Term Services and Supports (LTSS) Coordination Program Manager at BCIL is responsible for overseeing the LTSS Coordination team, which assists individuals with disabilities in OneCare health plans. This role involves managing day-to-day operations, facilitating team meetings, supervising staff, and maintaining a consumer caseload to help clients maximize their independence and well-being. The Program Manager also serves as a member of the senior management team and is committed to promoting diversity and inclusion within the organization.

Responsibilities

  • Manage day-to-day LTSS coordination operations, including staff training and supervision.
  • Oversee the consumer intake process and assignment of cases.
  • Ensure timely documentation of work and submission of reports.
  • Facilitate team meetings and assist with the orientation of new staff.
  • Identify training opportunities to enhance team and individual skills.
  • Conduct regular performance reviews for staff members.
  • Develop and maintain relationships with other agencies and organizations that assist consumers.
  • Represent BCIL at public events and collaborate with other departments as needed.
  • Assess the LTSS needs of consumers and advocate for them.
  • Coordinate services and provide education on long term services and supports to consumers and their care teams.
  • Arrange and coordinate the authorization and provision of community long term services and supports.

Requirements

  • At least three years of relevant experience working with people with disabilities.
  • Bachelor's degree in a relevant field.
  • Supervisory experience and skills in conducting meetings and enhancing staff knowledge.
  • Knowledge of the home and community-based service system.
  • Experience in conducting assessments for LTSS needs and monitoring service delivery.
  • Cultural competence and informed advocacy skills based on independent living principles.
  • Ability to develop Individualized Care Plans and communicate effectively across service systems.
  • Strong writing, organizational, and computer skills.
  • Ability to work independently and as part of a team.
