About the position

The Licensed Practical Nurse (LPN) in the Academic Accountable Care Unit at Mount Carmel St. Ann's is responsible for providing administrative support and patient care in a fast-paced, collaborative environment. This role involves working with a diverse patient population, including those with medical/surgical, telemetry, and critical care needs, while focusing on individualized and evidence-based care. The LPN will collaborate with various healthcare professionals to ensure optimal patient outcomes and participate in the educational development of both patients and staff.

Responsibilities

• Directs unlicensed staff in work-related activities under the supervision of a registered nurse.
• Uses assessment skills to collect data and collaborates with RN to discuss next steps in patient care.
• Communicates pertinent clinical information to appropriate members of the patient care team and keeps patients/families updated.
• Provides patient/family education under the direction of the registered nurse.
• Identifies discharge needs in conjunction with the physician and RN, participates in planning, and completes continuity of care forms as appropriate.
• Collaborates with attending physicians, house staff, registered nurse, and other healthcare disciplines regarding patient care.
• Performs treatments and procedures according to approved standards, policies, procedures, and guidelines.
• Documents accurately and completely pertinent information/data in accordance with documentation standards, policies, procedures, and/or guidelines.
• Utilizes computer systems effectively, including ATK and Registrar, and appropriate use of Internet/Intranet.
• Takes responsibility to facilitate improvements in patient services, staff/physician relationships, and the working climate.

Requirements

• Successful completion of an approved Pharmacology Course with NAPNES Certification, OOPNES Certification or Ohio Board of Nursing Card.
• Graduate of an accredited School of Practical Nursing.
• Current license in good standing to practice as a Licensed Practical Nurse (LPN) in the State of Ohio.
• Current BLS/CPR certification and maintain current certification; IV certification preferred.
