About the position

The LPN (Licensed Practical Nurse) position at Van Dyk Health Care is a full-time role focused on providing nursing care in an assisted living environment. The LPN will implement nursing objectives, make independent care decisions, and maintain confidentiality while supervising medication administration and performing routine charting duties. This role is essential in ensuring the well-being of residents and involves collaboration with physicians and family members regarding changes in resident conditions.

Responsibilities

• Implement and maintain established nursing objectives and standards.
• Make independent decisions concerning nursing care.
• Notify resident's physician and next of kin/sponsor when there is a change in the resident's condition.
• Maintain confidentiality of all resident care information.
• Supervise the self-administration of medications as ordered by the physician.
• Administer medications and monitor resident conditions.
• Dispose of drugs and syringes as required, in accordance with established procedures.
• Receive telephone orders from physicians and record on telephone order form to be signed and returned within 48 hours.
• Transcribe physicians' orders to pharmacy order sheets, lab requests forms, and MAR (Medication Administration Record).
• Perform routine charting duties in accordance with established procedures.
• Sign and date all entries in resident's medical file.
• Chart all accidents/incidents involving residents and follow established procedures.
• Perform day-to-day functions of nursing activities in accordance with current rules, regulations, and guidelines that govern the facility.
• Perform administrative duties such as completing medical forms, reports, studies, and charting as necessary.
• Participate in surveys (inspections) made by authorized government agencies.

Requirements

• Mid-level LPN qualification
• Experience in nursing care within assisted living or similar environments

Nice-to-haves

• Experience with electronic medical records (EMR)
• Certification in CPR and First Aid
