About the position

The LVN Case Manager at Optum is responsible for performing clinical operations and medical management activities across the continuum of care. This role involves assessing, planning, implementing, coordinating, monitoring, and evaluating patient care, while working collaboratively within an interdisciplinary team. The position emphasizes health education, coaching, and treatment decision support for members, contributing to the overall goal of improving healthcare for communities.

Responsibilities

• Perform clinical operations and medical management activities across the continuum of care.
• Conduct case management and coordination of care.
• Provide medical management consulting within an interdisciplinary team.
• Deliver health education and coaching to members.
• Support treatment decision-making for patients.

Requirements

• Current unrestricted NY LVN license
• 2+ years of experience in a physician's office, clinical or hospital setting
• Knowledge of chronic diseases, especially COPD/asthma, diabetes, CHF and IHD
• Proficient computer skills to work efficiently with electronic medical records
• Ability to travel within a commutable distance of Poughkeepsie for internal and external meetings.

Nice-to-haves

• Experience related to patient education and/or motivational interviewing skills and self-management goal setting
• Case management experience
• Comfortable speaking and presenting to hospital partners and executive leadership.
