About the position
The LPN, Continuous Care role at Chapters Health involves providing nursing interventions and education to enhance patient comfort and quality of life under the supervision of an RN. This position is integral to delivering outstanding patient care and customer service in the community, reflecting the company's mission-driven approach.
Responsibilities
- Provide nursing tasks to patients and families such as continuous care, dressing changes, and catheter changes as per the plan of care developed by the Interdisciplinary Group (IDG).
- Demonstrate clinical skills in medication administration, symptom management, and patient/family relationships.
- Monitor patients for alterations in body systems, pain, and symptoms, and report findings to the RN.
- Observe and report signs of adverse reactions and responses to treatments to the RN.
- Perform venipuncture for blood specimens and manage Central Venous Line (CVL) site inspections and dressing changes.
- Document patient care activities per policy and procedure in the medical record.
- Participate in emergency response as required, including reporting to work during emergencies.
- Cross-train to Hospice House positions when necessary.
Requirements
- High school diploma or G.E.D. or equivalent combination of education and experience.
- Current LPN licensure in the state of employment.
- Active BLS certification for healthcare professionals from the American Heart Association or Red Cross.
- Valid driver's license and automobile insurance per company policy.
- Reliable transportation to meet visit schedule.
- Certificate of completion of 30-hour Infusion Therapy Course and in-house 8-hour Infusion Class preferred for inpatient unit work.
- Ability to communicate effectively in English, both written and verbal.
- Flexible schedule to meet patient needs.
- Ability to manage the emotional stress of caring for terminally ill patients.
Nice-to-haves
- Experience in hospice or palliative care settings.
- Additional certifications in nursing specialties.
