About the position

The LPN (Continuous Care) at LifePath Hospice plays a crucial role in providing nursing interventions and teaching skills to enhance patient comfort and quality of life under the supervision of an RN. This full-time position involves working closely with patients and their families, ensuring high-quality care and support in a hospice setting.

Responsibilities

• Provide nursing tasks to patients and families such as continuous care, dressing changes, and catheter changes as per the care plan.
• Demonstrate clinical skills in medication administration, symptom management, and patient/family relationships.
• Monitor patients for alterations in body systems, pain, and symptoms, and report findings to the RN.
• Perform venipuncture for blood specimens and manage Central Venous Line (CVL) site observations and dressing changes.
• Document patient care activities in the medical record according to policy and procedure.
• Participate in emergency response as required, including reporting to work during emergencies.
• Engage in quality assessment and performance improvement activities.

Requirements

• Current LPN licensure in the state of employment.
• Active BLS certification for healthcare professionals from the American Heart Association or Red Cross.
• High school diploma or GED or equivalent combination of education and experience.
• Ability to communicate effectively in English, both written and verbal.
• Flexible schedule to meet patient needs.
• Ability to manage emotional stress associated with caring for terminally ill patients.

Nice-to-haves

• Certificate of completion of a 30-hour Infusion Therapy Course and in-house 8-hour Infusion Class preferred.
• Experience in teaching.
