About the position

The LPN Continuous Care position at Empath Health is a full-time role that requires a dedicated and compassionate Licensed Practical Nurse to provide care in Pinellas County. The working hours are from 4 PM to 12 AM, with flexible days and a weekend requirement. As part of the team, the LPN will collaborate with other healthcare professionals to develop and implement individualized care plans for patients. This role requires excellent communication skills to effectively interact with patients, families, and healthcare providers. The LPN will also be expected to demonstrate strong critical thinking and multi-tasking abilities, as they will often need to manage multiple patient needs simultaneously. Responsibilities

• Provide nursing care to patients in their homes during the evening shift.
• Collaborate with healthcare professionals to develop and implement individualized care plans.
• Maintain accurate and up-to-date patient records and documentation.
• Communicate effectively with patients, families, and healthcare providers regarding patient care.
• Adhere to HIPAA regulations and maintain patient confidentiality.
• Participate in the seasonal influenza program as a condition of employment.

Requirements

• Graduate of a licensed certified Practical Nursing program.
• At least 1 year of clinical experience as an LPN.
• Maintain LPN license in accordance with State of Florida requirements.
• Valid CPR certification from the American Red Cross or American Heart Association.
• Current IV Therapy certification preferred or willingness to obtain within 6 months.
• Proof of valid Florida State Driver's License and valid automobile insurance.
• Excellent verbal and written communication skills.
• Strong multi-tasking and critical thinking skills.
• Proficient typing and computer input skills.
• Knowledge of Microsoft Office products (MS Outlook, Word, Excel); knowledge of Suncoast Solutions preferred.
• Understanding of HIPAA compliance regulations.
