About the position

The Continuous Care Licensed Practical Nurse (LPN) position at Empath Health is a full-time role that requires working three 12-hour shifts, with a weekend requirement. The specific days of the week for these shifts will be determined based on the needs of the organization. The coverage area for this position includes Manatee, Charlotte, De Soto, and Sarasota County. Under the supervision of a Registered Nurse (RN) and following physician orders, the LPN will be responsible for observing patient and family needs, providing supportive care, and offering emotional support to patients and their families during times of crisis. This role is crucial in maximizing patient comfort and enhancing their quality of life. The LPN will perform continuous bedside clinical observations and provide prescribed care as outlined in the patient's plan of care. They will report any significant changes in the patient's condition and family needs to the RN case manager. Additionally, the LPN will collaborate with the RN to accept and process physician orders to ensure the highest level of patient comfort. Crisis Care nurses in this role are also cross-trained to assist in Hospice Houses and Triage, providing support for patient care needs when cases are pending. Responsibilities

• Provide continuous bedside clinical observations and prescribed care to patients under the direction of the RN case manager.
• Report significant changes in patient condition and family needs to the RN as needed.
• Collaborate with the RN to accept and process physician orders to maximize patient comfort.
• Assist with patient care needs in Hospice Houses and Triage when cases are pending.

Requirements

• Graduate of an approved nursing program with a current Florida LPN license.
• Minimum one year of experience in an acute care setting providing direct patient care.
• Previous experience as a hospice LPN is preferred.
