About the position

The Continuous Care (CC) Licensed Practical Nurse (LPN) position at Empath Health is a part-time role that requires working three 12-hour shifts, specifically during the PM shift from 8 PM to 8:30 AM, with alternating weekends. This role is crucial in providing bedside clinical observations and supportive care to patients and their families under the supervision of a Registered Nurse (RN) and following physician's orders. The LPN will be responsible for observing patient and family needs, offering emotional support during times of crisis, and ensuring that patient comfort is maximized to enhance their quality of life. In this position, the LPN will work closely with the RN case manager to deliver prescribed care and report any significant changes in the patient's condition or family needs. The role emphasizes collaboration with the RN to accept and process physician orders effectively, ensuring that the care plan is followed and adjusted as necessary to meet the evolving needs of the patient. Responsibilities

• Under the direction of the RN case manager, provides continuous bed-side clinical observations, prescribed care, physical, and emotional support to patients as directed in plan of care, reporting significant changes in patient condition and family needs as needed.
• In collaboration with the RN, accepts and processes physician orders to maximize patient comfort.

Requirements

• Applicants must be a graduate of an approved nursing program and own a current Florida LPN license.
• Minimum one year experience in an acute care setting, providing direct patient care; previous experience as a hospice LPN preferred.
