About the position

The LPN Continuous Skilled Nursing position at Northeast Arc involves providing compassionate and skilled nursing care to medically fragile pediatric and adult patients in their homes, schools, or community settings. The role focuses on ensuring patients receive continuous care, helping families navigate complex medical needs, and preventing hospitalizations. Nurses will work with a diverse range of medical conditions, from general nursing care to complex ICU-level home care, and will be part of a supportive team dedicated to high-quality patient care.

Responsibilities

• Provide one-on-one nursing care to patients in home and community settings.
• Administer medications and treatments as prescribed by healthcare providers.
• Monitor patients' health status and report any changes to the healthcare team.
• Collaborate with families and interdisciplinary teams to develop and implement care plans.
• Educate patients and families on managing medical conditions and care needs.
• Maintain accurate and timely documentation of patient care activities.

Requirements

• Valid LPN license in the state of Massachusetts.
• Proficiency in nursing skills and critical thinking.
• Experience in handling diverse medical conditions is beneficial, though new graduates are welcome.
• Compassionate and empathetic approach to patient care.
• Ability to adapt to varying patient needs and environments.
• Strong communication skills for effective collaboration with patients, families, and healthcare teams.
• Professionalism, including punctuality and reliability.
• Cultural sensitivity and respect for diverse backgrounds.

Nice-to-haves

• Experience with complex medical conditions such as tracheostomies and ventilator support.
• Familiarity with feeding challenges and G or J-tube feeding.
• Previous experience in home healthcare settings.
