About the position

We are seeking a highly motivated and self-directed Licensed Practical Nurse (LPN) for mobile visits in the East Valley area, including Mesa, Chandler, and Tempe. This role focuses on providing home health care, particularly in ultrasound therapy and wound care, with training provided for the right candidate. The position operates during Monday to Friday business hours, with no weekends or holidays required, and involves visiting patients in their home settings.

Responsibilities

• Plans and manages patient care according to each patient's needs.
• Interviews patients and records their medical history and physical condition.
• Performs non-contact, non-thermal ultrasound per instructions and under supervision.
• Obtains patient vital signs, including pulse, blood pressure, temperature, and respiration.
• Participates in coordination and execution of telehealth appointments as needed.
• Provides routine wound care for patients.
• Observes patients' health and reviews/document in medical charts.
• Assists clinician providers and office staff.
• Communicates with patients and their families.
• Performs standard diagnostic procedures including EKG, phlebotomy, venipuncture, PFT, and Doppler.
• Responsible for medical equipment and supplies.
• Prepares specimens for lab courier.
• Administers medications and provides wound care.
• Assists with patient comfort and performs other duties as assigned.

Requirements

• Completion of state approved training program (diploma, certificate, or associate degree).
• Valid LPN license (pass NCLEX-PN exam).
• Experience in home health and wound care.
