About the position

At The Cancer & Hematology Centers, we are dedicated to providing help, healing, and hope for cancer patients and their families. As a Licensed Practical Nurse (LPN), you will play a crucial role in ensuring that our patients receive the highest quality of care in a supportive environment. This position can be either full-time or part-time, with part-time hours consisting of 4-5 partial days per week. In this role, you will work closely with a team of healthcare professionals, including physicians, oncology-certified nurses, social workers, and pharmacy personnel, to address the needs of our patients. You will be responsible for handling telephone messages from patients, hospitals, physicians, and pharmacies regarding patient care, medication refills, and the overall plan of care. Your duties will also include conducting physical assessments, administering injections, and managing follow-up plans and appointments. Your contributions will not only focus on the clinical aspects of care but also on providing emotional support to patients and their families during a challenging time. Responsibilities

  • Handle telephone messages from patients, hospitals, physicians, pharmacies, etc. regarding patient care and involving plan of care, disease process and medication refills in conjunction with physicians and registered nurses.
  • Work through inbox and canopy.
  • Staff injection clinic by doing physical assessments, assessing laboratory values, administering injections, and establishing follow-up plan and future appointments.
  • Refill non-oncolytic, and unscheduled medications per policy.
  • Follow patient plan of care established by physician, Nurse practitioner or Registered Nurse.
  • Function as a team member by expanding responsibilities as directed by management to facilitate optimal workflow within the cancer center.
  • Give clinical support to the scheduling department so appropriate tests are set up per physician request.
  • Give support to infusion MA role as needed.
  • In collaboration with pharmacy, nursing, and social work, manage patients who are receiving medicines via patient assistance programs and/or patients required to enroll in medication management for oral meds.

Requirements

  • Licensed LPN in the State of Michigan.
  • Preferred oncology outpatient or acute care experience.
