Pay $24.72 - $32.22 an hour Job type Full-time Shift and schedule No weekends Monday to Friday Weekends & holidays off Full job description Schedule: Monday - Friday | 8:00am - 4:30pm | No weekends Hours: 40 hours per week Location | Department: LifeCircles PACE | Muskegon Overview: The Home Care Coordinator Level 2 works in conjunction with the interdisciplinary team (IDT) to assess and implement home care services, including coordination of specified durable medical equipment, supplies, and in home personal care, homemaking, respite, and community living services. What you'll do: Perform evaluation of participant's goals and needs. Provide/arrange assistance for participant's ADL and IADL deficits in the home environment. Provide/arrange home respite care to reduce caregiver stress levels. Ensures all durable medical equipment is understood by participant and/or caregiver. Facilitate coordination of home exercise programs with therapy and home health aides. Coordinate and provide medical supplies. Provide medication reminders in the home setting. Assist the IDT in the development of an emergency plan of care in the home setting. Conduct regular home visits. Regularly communicate the appropriateness of the plan of care to the IDT and any change in condition or need. Bi-directional communication of the care plan. Monitor care plan, ensuring it is being followed. What it takes: A current active Practical Nurse License (LPN) in the State of Michigan. 3 years of experience as an LPN in a geriatric setting is preferred. 1 year experience working with the frail or elderly population. Reliable transportation and a current valid Michigan driver's license, required. Current CPR certification by AHA or willing and able to obtain as offered by LifeCircles required.
