About the position

The Licensed Practical Nurse (LPN) will provide direct patient care as part of a treatment team, working under the supervision of a Registered Nurse and attending clinician. The role involves planning, implementing, and evaluating nursing care for assigned patients, focusing on bedside care, medication administration, and patient monitoring.

Responsibilities

  • Administers oral and IV medications according to the patient's treatment plan and as ordered by the attending clinician, under RN supervision.
  • Assists in IV Medication Administration and Maintenance Therapy, including validating solutions, analyzing infusion sites, and monitoring patient comfort.
  • Supervises and participates in care administered by Patient Care Techs, including obtaining vital signs and assisting with basic patient needs.
  • Provides direct care to patients, carrying out their plans of care and observing their behavior and responses to treatments.
  • Monitors patient vital signs and reports observations to the RN.
  • Recognizes deterioration in patient condition and collaborates with the RN for appropriate responses.
  • Performs phlebotomy as ordered, ensuring correct patient identification and proper labeling of samples.
  • Conducts bedside glucose monitoring as appropriate.
  • Implements preventative care and performs wound treatments as per the individualized treatment plan.
  • Provides patient and family health education based on assessed needs.

Requirements

  • Technical diploma in nursing
  • 30-hour IV Certification
  • 1 year of LPN experience

Nice-to-haves

  • Experience in home health care
