About the position

As a Home Health LPN at CenterWell Home Health, you will provide professional nursing services under the direction of a Registered Nurse (RN) while adhering to state regulations and healthcare standards. Your role will involve delivering skilled nursing interventions, coordinating care, and maintaining communication with patients and healthcare teams to ensure optimal patient outcomes. You will also be responsible for documenting care and maintaining confidentiality in all patient interactions.

Responsibilities

  • Provide professional nursing services under the direction of an RN in compliance with state regulations and healthcare standards.
  • Deliver skilled nursing interventions for patient treatment, rehabilitation, and preventative care.
  • Utilize a holistic approach to implement therapeutic interventions and training activities according to the care plan.
  • Confer with the RN Supervisor regarding changes in the Plan of Treatment and communicate physician orders as permitted.
  • Maintain timely communication with patients, physicians, and healthcare team members regarding patient needs and status changes.
  • Participate in care coordination activities and discharge planning as appropriate.
  • Maintain confidentiality and professional conduct in all patient interactions.
  • Document care provided and maintain required documentation as per company policies.

Requirements

  • Graduate of an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing.
  • Current nursing license in the practicing state.
  • Valid driver's license, auto insurance, and reliable transportation.
  • Current CPR certification.
  • Two years of experience as an LPN/LVN in a clinical setting, preferably in home health or hospice.
