About the position
As a Home Health LPN at CenterWell, you will provide professional nursing services under the direction of a Registered Nurse (RN) while ensuring compliance with state regulations and healthcare standards. Your role will involve delivering skilled nursing interventions, coordinating care, and maintaining communication with patients and healthcare teams to achieve individualized patient goals. This position emphasizes a holistic approach to patient care, focusing on rehabilitation and preventative measures.
Responsibilities
- Provide professional nursing services under the direction of an RN in compliance with state regulations.
- Deliver skilled nursing interventions for patient treatment and rehabilitation needs.
- Utilize a holistic approach in providing therapeutic interventions and training activities.
- Apply clinical knowledge and skills to facilitate problem resolution and achieve patient goals.
- Confer with the RN Supervisor regarding changes in the Plan of Treatment.
- Accept verbal orders from physicians as permitted by state law and communicate these to the RN Supervisor.
- Implement the nursing plan of care using a systematic, individualized approach.
- Maintain timely contact with patients, physicians, and healthcare team members regarding patient needs and status changes.
- Participate in care coordination activities and discharge planning as appropriate.
- Maintain confidentiality and adhere to professional standards in communication and documentation.
Requirements
- Graduate of an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing.
- Current nursing license in the practicing state.
- Valid driver's license, auto insurance, and reliable transportation.
- Current CPR certification.
- Two years of experience as an LPN/LVN in a clinical setting, preferably in home health or hospice.
