About the position
As a Home Health LPN at CenterWell Home Health, you will provide professional nursing services under the direction of a Registered Nurse (RN) while ensuring compliance with healthcare standards and regulations. This role involves delivering skilled nursing interventions, maintaining patient communication, and participating in care coordination activities to support patients in their recovery and rehabilitation process.
Responsibilities
- Provide professional nursing services under the direction of an RN in compliance with the state's Nurse Practice Act and applicable healthcare standards.
- Deliver skilled nursing interventions for patient illness, rehabilitation needs, and preventative care.
- Utilize a holistic approach in providing therapeutic interventions and teaching activities according to the plan of care.
- Confer with the RN Supervisor regarding changes in the Plan of Treatment and communicate physician orders as permitted by state law.
- Implement the nursing plan of care using a systematic, individualized goal/outcome driven approach.
- Maintain timely communication with patients, physicians, and healthcare team members regarding patient needs and status changes.
- Participate in care coordination activities and discharge planning as appropriate.
- Maintain confidentiality and adhere to professional standards in sharing patient information.
- Prepare, submit, and maintain required documentation, including visit notes.
Requirements
- Graduate of an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing.
- Current nursing license in the practicing state.
- Valid driver's license, auto insurance, and reliable transportation.
- Current CPR certification.
- Two years of experience as an LPN/LVN in a clinical setting, preferably in home health or hospice.
