About the position

The LPN position at Enhabit Home Health & Hospice involves providing nursing care to patients in their homes under the direction of Registered Nurses (RNs) and in compliance with physicians' orders. This role is crucial in delivering high-quality home-based care, contributing to the overall mission of enhancing patient care while fostering a collaborative and innovative workplace culture.

Responsibilities

• Visit patients in their homes to provide nursing care.
• Administer medications and treatments as prescribed by physicians.
• Monitor patients' health status and report changes to RNs.
• Assist with daily living activities and provide emotional support to patients and families.
• Document patient care and maintain accurate medical records.

Requirements

• Must possess a valid state driver's license.
• Must be currently licensed as a Licensed Practical Nurse in the State of employment.
• Dependable transportation kept in good working condition.
• Must possess automobile liability insurance.
• Must be able to drive an automobile in all types of weather conditions.

Nice-to-haves

• Experience in home health care or similar settings.
