About the position
The LPN - Home Visits position involves providing practical nursing care to patients in their homes under the supervision of a Registered Nurse. The role focuses on delivering health education, routine nursing care, and monitoring patient conditions while ensuring proper documentation and communication with healthcare professionals.
Responsibilities
- Administer practical nursing care to patients according to physicians' orders under the supervision of a Registered Nurse.
- Provide health education based on the teaching plan established by the RN.
- Deliver routine LPN nursing care as outlined in the nursing care plan.
- Observe and report signs and symptoms, patient reactions to treatments, and changes in the patient's physical or emotional condition to the physician and RN Case Manager.
- Document services rendered to the patient and any changes in the patient's condition or family and home situations.
- Document communication with the RN Case Manager and physician.
- Perform other duties as assigned.
Requirements
- Licensed Practical Nurse (LPN) certification.
- Experience in home healthcare or similar nursing environment.
- Ability to work under the supervision of a Registered Nurse.
