About the position

The LPN Inpatient Case Manager position is a hybrid role based in Riverside, CA, focused on providing a holistic, patient-centered approach to care. The Case Manager will work under the supervision of an RN or MD, performing clinical operations and medical management activities across the continuum of care. Responsibilities

• Identify, coordinate and/or provide appropriate levels of care under the direct supervision of an RN or MD
• Perform clinical operations and medical management activities across the continuum of care, including assessing, planning, implementing, coordinating, monitoring, and evaluating
• Provide health education, coaching, and treatment decision support for members

Requirements

• High School diploma or GED or equivalent work experience
• Unrestricted LPN or LVN license in the State where the position will be performed
• 3+ years case management or acute care experience
• Basic computer proficiency (Microsoft Word, Outlook and internet)
• Ability to travel 30-50% of the time to local hospitals on a daily basis
