About the position
Citizens Memorial Hospital Foundation (CMH) is seeking a dedicated Licensed Practical Nurse (LPN) to join our team at the Lake Stockton Healthcare Facility, a skilled nursing facility located in Stockton, MO. This facility is equipped with 90 beds and includes specialized memory care units designed for individuals suffering from Alzheimer's disease and dementia. As an LPN, you will play a crucial role in providing high-quality care to our residents, working closely with a supportive healthcare team to ensure their well-being and comfort. In this position, you will have the opportunity to build meaningful relationships with residents while utilizing your strong critical thinking and assessment skills. Your responsibilities will include safe medication administration, conducting thorough resident assessments, and monitoring changes in Activities of Daily Living (ADLs). You will also be tasked with administering treatments and procedures as per physician orders. The Charge Nurse will assign care duties to nursing personnel based on their training and experience, and you will be an integral part of the interdisciplinary team that facilitates resident admissions and discharges, as well as the development of individualized Plans of Care. This is a full-time position with day shifts from 6 AM to 6 PM, allowing you to engage in a rewarding career that makes a difference in the lives of our residents. Responsibilities
- Provide excellent care to residents in accordance with federal and state regulations.
- Administer medications safely and effectively.
- Conduct resident assessments and monitor changes in Activities of Daily Living (ADLs).
- Administer treatments and procedures as per physician orders.
- Collaborate with the Charge Nurse and interdisciplinary team for resident admissions and discharges.
- Develop and implement individualized Plans of Care for residents.
Requirements
- Associates degree in nursing or related field.
- Current LPN license to practice nursing.
- Strong critical thinking and assessment skills.
- Ability to provide patient care with compassion and professionalism.
Nice-to-haves
- Experience in long-term care or skilled nursing facilities.
- Familiarity with memory care for Alzheimer's and dementia patients.
