- Maintain accurate patient records and documentation.
- Collaborate with other healthcare professionals to ensure comprehensive care.
- Participate in patient care planning and evaluation.
Requirements
- Valid LPN license required.
- BLS Certification required.
- Minimum of 1 year of experience in skilled long-term care or transitional care.
Nice-to-haves
