About the position

South Coast Post Acute is seeking a Licensed Vocational Nurse (LVN) / Licensed Practical Nurse (LPN) to join their team. This role focuses on providing high-quality care to residents in a skilled nursing facility, ensuring their physical and emotional well-being under the supervision of a registered nurse. The position offers both full-time and part-time opportunities with shifts available from 3pm to 11:30pm and 11pm to 7:30am.

Responsibilities

• Admitting, transferring, and discharging residents.
• Conducting daily rounds to observe the resident's physical and emotional status.
• Administering medications.
• Monitoring resident care provided by unlicensed staff.
• Maintaining the confidentiality of all resident care information.
• Promoting a culture of safety to ensure a healthy practice and living environment.

Requirements

• Current LVN / LPN licensure to practice in the state.
• Strong communication and organizational skills.
• Basic knowledge of computer use or willingness to learn.

Nice-to-haves

• Experience in a Skilled Nursing Facility (SNF) environment.
• Nursing experience providing direct care to multiple residents.
• Behavioral care / memory care experience.
• Experience with Point Click Care (PCC).
