About the position

The Licensed Practical Nurse (LPN) in Mobile Crisis Services plays a crucial role in providing immediate medical assessment and care to individuals in crisis. Working as part of an interdisciplinary team, the LPN is responsible for outreach, crisis stabilization, and linking clients to necessary services. This position requires adherence to Pennsylvania's Nurse Practice Act and Elwyn's policies, ensuring quality care and effective communication with other healthcare providers.

Responsibilities

  • Complete a focused nursing evaluation at the site of the encounter, including medical history and current concerns.
  • Engage emergent resources if there is an escalation or imminent danger during the initial encounter.
  • Document all details of the encounter in the client's chart and electronic record promptly.
  • Facilitate transfer to the hospital/Emergency Department by communicating details of the encounter with emergency services.
  • Collaborate with the client's mental health and primary care resources to implement the therapeutic program of care.
  • Present pertinent data obtained during the client encounter to the Interdisciplinary Team (IDT).
  • Report any observed medication and treatment side effects to the Registered Nurse (RN).
  • Administer and document medications as indicated, ensuring necessary consents are completed.
  • Document all behavioral and physical changes in the client's status in the nursing progress notes.
  • Order and restock medical supplies as needed.
  • Maintain medical equipment and ensure proper infection control practices.
  • Demonstrate flexibility and teamwork in nursing assignments to ensure quality care.
  • Adhere to HIPAA policies and maintain confidentiality of client records.
  • Complete mandatory educational requirements and attend nursing meetings as required.
  • Submit required documentation such as nursing license and CPR certification on time.
  • Perform CPR, first aid, and crisis intervention as needed.

Requirements

  • Licensed as a Licensed Practical Nurse in Pennsylvania.
  • One year of psychiatric, medical surgical, or chemical dependency nursing experience.
  • Knowledge of program regulating bodies preferred.
  • Ability to work in a fast-paced environment and manage multiple priorities.
  • Strong attention to detail and time management skills.
  • Excellent customer interaction and communication skills.
  • Basic experience with Microsoft Office applications.
  • Knowledge of Electronic Health Record (EHR) / Electronic Medical Record (EMR) preferred.
  • Current valid driver's license and acceptable driving record.

Nice-to-haves

  • Experience in crisis intervention techniques.
  • Familiarity with community health resources and services.
