About the position

The Licensed Practical Nurse (LPN) in Community Support Services at South Jersey Behavioral Health Resources, Inc. is responsible for providing nursing services and education to consumers with severe and persistent mental illness. This full-time position involves working under the supervision of the Director of Nursing, delivering direct nursing care, and supporting the mental health rehabilitation of clients in the community. New graduate LPNs are encouraged to apply, and the role offers a competitive hourly wage with potential for loan forgiveness.

Responsibilities

• Provide a minimum of 5 hours daily of direct face-to-face nursing and/or mental health rehabilitation services to consumers.
• Assess the medical needs of each consumer and coordinate care with treating providers.
• Administer medications and educate consumers about their medications and health-related issues.
• Monitor consumer side effects and report any problematic reactions to the supervisor or treating physician.
• Prepare clear, sound, accurate, and informative documentation of all consumer interactions and corresponding follow-up activities.
• Conduct monthly clinical record audits and review labs and medical records, reporting abnormal findings to the supervisor and ordering physician.

Requirements

• Current New Jersey licensure as an LPN is required.
• A valid driver's license is required, with a driver's abstract meeting insurance carrier's requirements.
• Graduation from an accredited school of practical nursing is required.
• Two years of licensed nursing experience and mental health experience is preferred.
