About the position

The LPN (Licensed Practical Nurse) in the orthopedic practice setting at Oneida Health is responsible for collaborating with the clinical team to provide high-quality patient care. This role involves various clinical tasks, patient education, and coordination of care to ensure exceptional patient experiences in a safe and efficient manner.

Responsibilities

  • Room patients and document chief complaints, vital signs, height, weight, and allergies.
  • Record and reconcile medications, including OTC drugs, and smoking status.
  • Identify barriers to medication adherence and treatment goals.
  • Review immunization status and follow up with patients post-ED or hospital admission.
  • Generate gap lists and schedule appointments for team patients.
  • Input recalls for needed services for team patients.
  • Liaise with after-hours care and ensure proper documentation in patient charts.
  • Respond to team patients' requests for clinical advice and document accordingly.
  • Assist providers (MD, PA, NP) with procedures and examinations, ensuring appropriate documentation is signed.
  • Attend to the needs of patients and visitors, providing reassurance and answering questions.
  • Create and update patient education materials and self-management tools.
  • Provide information on medications and collaborate with patients on treatment goals.
  • Follow procedures for authorized medication administration and documentation.
  • Perform treatments and procedures within the scope of training, including administering injectable medications.
  • Ensure patients are seen in an orderly manner and communicate delays to the team.
  • Maintain medical records, ensuring all reports and labs are filed/scanned prior to appointments.
  • Assist with coordinating care by tracking overdue labs and imaging, and notifying patients of test results.
  • Complete routine equipment checks and stock supplies in exam and treatment rooms.
  • Place orders from Central Supply and Pharmacy as necessary.
  • Participate in routine exam room cleaning and facilitate physician orders based on patient needs.
  • Coordinate and schedule referrals/surgeries with other specialists and ensure follow-up is arranged.
  • Prepare and send referrals with Continuity of Care Document (CCD) and inquire about self-referrals.
  • Provide training for clinical staff and perform secretarial duties when necessary.
  • Assist with insurance claims and pre-certification processes.
  • Respond to emergencies, assisting with resuscitation and CPR as needed, and ensure proper documentation of Code procedures.
  • Participate in quality improvement activities and attend daily huddles and monthly team meetings.
  • Communicate effectively with team members to ensure smooth workflow and meet patient needs.

Requirements

  • Graduation from a Licensed Practical Nurse Program registered by the New York State Education Department.
  • 1 year of experience in a healthcare environment.
  • LPN licensure in New York State.
