About the position
The LPN position involves working under the direction of the Hospice and Palliative Care Director, closely coordinating with the Palliative Nurse Practitioner (NP) and Palliative Medical Director to implement a comprehensive palliative care plan for patients. The role requires effective communication, strong clinical assessment skills, and proficiency in electronic medical record systems to ensure high-quality patient care.
Responsibilities
- Processing new referrals
- Maintaining the schedule for Palliative NP
- Entering encounters in Cerner EMR once a schedule is made
- Entering proposals for narcotics in EMR for MD to sign
- Tracking when narcotic scripts are needed
- Making weekly telephone calls to palliative patients or families
- Answering calls from palliative patients and triaging needs for Palliative NP
- Organizing the monthly palliative meeting, including scheduling and keeping notes
- Actively participating in team meetings
- Assisting with ordering DME, including faxing orders and following up on DME needs
- Assisting with obtaining prior authorizations for medications when required
- Coordinating with social workers for social needs
- Performing other related job duties as assigned
Requirements
- Completion of an accredited LPN Program
- Prefer at least one year of experience
- Excellent communication skills with patients and medical team members
- Strong clinical assessment skills
- Ability to work well with an electronic medical record system
- Maintaining a GA LPN License
