About the position

The LPN Patient Service Coordinator is a vital role within the healthcare team, serving as the first point of contact for patient care. This position involves a combination of administrative and patient care responsibilities, requiring independent initiative and judgment. The coordinator manages patient schedules, assists with infection control processes, and supports clinical staff during busy periods, ensuring efficient operations and high-quality patient care.

Responsibilities

• Manage schedules for all patients and edit schedules for agents calling in sick.
• Initiate infection control forms and send completed reports to HRD.
• Serve as backup during lunch hours and assist with calls from field staff.
• Maintain the client hospitalization log and send electronic logs to staff.
• Complete requested schedules for skilled nurse/injection visits and other tasks as they appear on the action screen.
• Run scheduling reports and prepare weekly agent schedules for approval.
• Verify visit paper notes in the scheduling console as needed.
• Assist with internal transfers of patients between branch offices.
• Receive lab reports, assess for normality, and route them appropriately.

Requirements

• Licensed Professional Nurse or Licensed Vocational Nurse in the state of practice.
• At least 1 year of home health experience.
• Excellent communication skills and ability to interact with diverse individuals.
• Strong organizational skills and ability to manage multiple assignments.

Nice-to-haves

• Prior packet review / QI experience preferred.
• Coding certification is preferred.
