About the position

As a Licensed Practical Nurse (LPN) at MedStar Health Home Care, you will play a crucial role in enhancing the quality of life for patients by providing direct and indirect care in their homes. This position emphasizes patient-centered care, allowing you to develop individualized care plans and support patients and their families in achieving health goals and independence. You will collaborate with a multidisciplinary team to ensure comprehensive care and maintain high standards of clinical practice.

Responsibilities

  • Develop and document individualized plan of care with specific care interventions and expected measurable patient outcomes.
  • Evaluate progress toward goals and revise plan of care as needed.
  • Establish, document, and implement personalized teaching plans for patients and their caregivers to help them achieve independence at home.
  • Perform nursing procedures consistent with homecare protocols.
  • Initiate and participate in regularly scheduled case conferences, including patient and family in mutual goal setting and care plan revision.
  • Maintain necessary clinical records, collect data, and prepare reports on activities.
  • Communicate regularly with the patient's physician, home healthcare team, and other key members of the care team.

Requirements

  • Graduation from an accredited program of Practical Nursing required.
  • 1-2 years of recent clinical experience in an acute care setting required.
  • Home care experience preferred.
  • Valid LPN license in the District of Columbia, Maryland or Virginia, or any combination as required based on work location(s) upon hire.
  • CPR certification upon hire required.
  • Valid State Driver's License in good standing upon hire required.

Nice-to-haves

  • Basic computer skills preferred.
  • Verbal and written communication skills.
