Affirmed Home Care is seeking qualified Licensed Practical Nurses (LPNs) to join their growing team in Bergen County, New Jersey. This part-time position offers flexibility in scheduling, allowing nurses to choose their hours while providing essential nursing care in patients' homes. The role emphasizes delivering high-quality nursing services and emotional support to clients and their families.

Responsibilities

• Traveling to patient's home and providing 'Best Practice' nursing care
• Checking vital signs
• Documenting in a complete and confidential manner
• Evaluating the patient's wellbeing and reporting any needs to the Director of Clinical Nursing or emergency services
• Providing personal care and meals
• Performing more advanced nursing tasks such as chest tubes, IVs, colostomy, and urinary catheter management
• Giving emotional support to clients and families in times of crisis

Requirements

• Valid LPN license
• Annual physical within one year
• PPD or Quantiferon within one year or chest x-ray within five years
• MMR labs
• COVID vaccine card with booster
