Responsibilities

• Demonstrates compassionate interpersonal and communication skills to serve patients and families professionally.
• Provides one-to-one patient care in a home environment.
• Assesses patients for stability and follows through with all doctor's orders.
• Verifies medication milligrams, last day dosed, take outs received, phase level, and verifies guest doses.
• Documents patient complaints, vital signs, current medication, and allergies accurately.
• Engages in health promotion, teaching, and training of family members as necessary.

Requirements

• High school diploma or equivalent and graduate of an accredited program.
• Must have a good standing LPN license in the state of practice.
• At least 12 months of hands-on nursing experience required.
• Current CPR certification is mandatory.
• Comfortable and efficient in the use of EMRs and computers.
• Experience with GTube, Trach/Vent is preferred.

Nice-to-haves

• Licensed or certified professionals preferred but willing to consider others with adequate experience.
