About the position
The LPN/RN position at Care Plus TLC involves providing essential nursing care to patients in a home health care setting. The role focuses on monitoring patient conditions, administering medications, and assisting with daily living activities, ensuring the highest standards of care and safety are maintained.
Responsibilities
- Take and record vital signs including fluid intake and output.
- Monitor patient's condition and report changes/concerns to appropriate medical professionals and family members.
- Provide Activities of Daily Living care as outlined in the Care Plan, including bathing, dressing, and feeding.
- Transfer patients with equipment using appropriate safety measures such as gait belts and lifts.
- Understand First Aid and Emergency Care protocols.
- Complete electronic documentation while on shift.
- Provide catheter care.
- Administer prescribed medications including injections and enemas/suppositories as ordered by the physician.
- Provide trach and vent care if applicable.
- Provide feeding tube care if applicable.
- Provide wound care if applicable.
Requirements
- Familiarity with Home Health Care, safety and sanitation standards and procedures.
- Understanding of Confidentiality and Privacy Laws.
- Excellent communication and interpersonal skills.
- Knowledge of basic medical terminology.
- Adequate knowledge of medical terminology.
- Ability to turn and reposition patients.
- Ability to undertake a variety of diverse care tasks and work as a team member.
