About the position

The Long Term Care LPN (Licensed Practical Nurse) position at Amergis in Jacksonville, NC, is a critical role responsible for the delivery of care to patients in a Long Term Care/Assisted Living Facility. The LPN will be accountable for making clinical judgments and coordinating patient care effectively and efficiently, under supervision. This position requires a strong ability to think critically and perform nursing duties according to established standards. The LPN will utilize the nursing process to assess, plan, implement, and evaluate patient care, ensuring that all actions align with the patient's needs and the facility's policies. In this role, the LPN will conduct focused interviews to identify specific patient needs, assess physiological and psychological changes in patients, and document nursing diagnoses and care plans. The nurse will monitor the consistency of the patient care plan with the actions of medical staff and provide interventions based on identified priorities. A safe and therapeutic environment for patients is paramount, and the LPN will initiate and evaluate patient and family education, revising care plans as necessary based on evaluations and changes in medical plans. The LPN will also be responsible for monitoring the safety and effectiveness of the environment and equipment, participating in activities that promote good interpersonal communication, and adhering to professional nursing standards and regulations. Responsibilities

  • Utilizes the nursing process to assess, plan, implement and evaluate patient care in a Long Term Care/Assisted Living Facility
  • Performs focused interview to identify specific needs
  • Assesses signs and symptoms indicating physiological and psychological changes in the patient's condition
  • Collects, analyzes, and interprets data and information from health care members and documents actual and/or potential nursing diagnoses
  • Documents the patient's plan of care using identified nursing diagnoses, expected patient outcomes, and selected nursing interventions
  • Identifies and prioritizes actions based upon patient care requirements
  • Monitors consistency between patient plan of care and actions of medical staff
  • Performs interventions according to identified priorities, plan of care, and the facility policies and patient outcome standards
  • Provides a safe, comfortable and therapeutic patient environment
  • Initiates and evaluates patient and/or family education
  • Revises the plan of care according to evaluation, changes in medical plan of care, and effective/ineffective nursing interventions
  • Documents patient's response to nursing and other interventions
  • Uses clinical judgment in evaluation activities to meet patient care needs of an assigned floor/unit including establishing priorities
  • Monitors safety and effectiveness of the environment and equipment
  • Participates in activities to facilitate good interpersonal communications with the chosen facility
  • Supports professional practice by observing the Nurse Practice Act, Standards of Nursing Practice, Standards of Patient Care, Joint Commission, state and local laws, Patient's Bill of Rights, and Amergis Healthcare policies
  • Performs other duties as assigned/necessary

Requirements

  • Current Licensed Practical Nurse License for the state in which the nurse practices
  • Ability to effectively elicit/provide information to and from appropriate individuals via strong spoken, written, listening, and nonverbal communication skills
  • Current Health Certificate (if applicable)
  • Current PPD or Chest X-Ray
  • One year prior professional nursing experience preferred
  • Current BLS card
  • Successful completion of screening tests for the clinical specialty area(s) in which the LPN practices with a score of 80% or greater
  • Successful completion of Background Screening Process
