About the position

The LTSS Service Care Manager is responsible for managing care for members with long-term support needs, primarily focusing on assessing and coordinating services for individuals who are aged, blind, or disabled. This role involves significant travel to meet members in their homes, conducting assessments, and ensuring that appropriate services are authorized and delivered. The position requires a blend of clinical knowledge and social service skills to advocate for members and facilitate their access to necessary resources and support.

Responsibilities

  • Conduct member assessments to determine eligibility for personal attendant services.
  • Document assessments and submit for service authorization or denial within a 3-business day turnaround.
  • Evaluate the needs of high-risk members and recommend appropriate care plans.
  • Act as a liaison and advocate for members, families, and healthcare providers.
  • Educate members and families on healthcare options, preventive care, and community resources.
  • Coordinate service authorizations for various support services based on assessments.
  • Maintain accurate documentation in the electronic care management system.
  • Provide feedback to leadership on quality improvement opportunities in care delivery.
  • Comply with all relevant policies and standards.

Requirements

  • 5+ years of experience working with the aged, blind, or disabled population.
  • Bachelor's degree with 2-4 years of related experience or LVN with 2-4 years of related experience.
  • Strong preference for LPN/LVN license or licensed social worker.

Nice-to-haves

  • Experience in social service, social work, care management, or service coordination.
  • Bilingual skills are strongly preferred.
