About the position

The LTSS Service Coordinator is a field-based role responsible for conducting service coordination functions for individuals in specialized programs. This position involves facilitating the Person Centered Planning process, conducting assessments, developing comprehensive support plans, and managing the physical and behavioral health needs of members. The coordinator acts as a single point of contact for care coordination, ensuring effective utilization of long-term services and supports while collaborating with various stakeholders to meet the members' goals and needs.

Responsibilities

  • Conduct face-to-face program assessments using various tools and motivational interviewing techniques.
  • Coordinate and manage an individual's waiver and behavioral/physical health needs.
  • Identify members with potential clinical healthcare needs and coordinate their cases with the clinical healthcare management team.
  • Manage non-clinical needs of members with chronic illnesses and disabilities to ensure efficient utilization of services.
  • Document short and long-term service and support goals in collaboration with the member's care team.
  • Identify members that would benefit from alternative levels of service or other waiver programs.
  • Serve as a mentor or subject matter expert for new staff and assist in training associates.
  • Submit utilization/authorization requests to management with supporting documentation.
  • Report critical incidents to appropriate internal and external parties.
  • Assist in appeals, grievances, and state audits.

Requirements

  • BA/BS degree and a minimum of 2 years of experience working with a social work agency.
  • Equivalent combination of education and experience may be considered based on state law and contract requirements.

Nice-to-haves

  • BA/BS degree in a healthcare-related field preferred.
  • Bilingual in English/Spanish strongly preferred.
