About the position
The LTSS Service Coordinator is responsible for conducting service coordination functions for a defined caseload of individuals in specialized programs. This role involves facilitating the Person Centered Planning process, conducting assessments, developing comprehensive support plans, and managing the physical and behavioral health needs of individuals in accordance with state and federal requirements.
Responsibilities
- Conduct face-to-face program assessments using various tools and motivational interviewing techniques.
- Coordinate and manage an individual's waiver and behavioral/physical health needs.
- Identify members with potential clinical health care needs and coordinate their cases with the clinical healthcare management team.
- Manage non-clinical needs of members with chronic illnesses and disabilities to ensure efficient utilization of long-term services and supports.
- Document short and long-term service and support goals in collaboration with the member's care team.
- Identify members that would benefit from alternative levels of service or other waiver programs.
- Serve as a mentor or subject matter expert for new staff and assist in training associates.
- Submit utilization/authorization requests to utilization management with supporting documentation.
- Report critical incidents to appropriate internal and external parties.
- Assist in appeals, member grievances, and state audits.
Requirements
- BA/BS degree and a minimum of 2 years of experience working with a social work agency, or equivalent education and experience.
Nice-to-haves
