About the position
The LTSS Service Coordinator is responsible for conducting service coordination functions for a defined caseload of individuals in specialized programs. This role involves facilitating the Person Centered Planning process, conducting assessments, and developing comprehensive support plans while managing the physical and behavioral health needs of individuals in accordance with state and federal requirements.
Responsibilities
- Conduct face-to-face program assessments to identify individual needs using various tools and motivational interviewing techniques.
- Coordinate and manage cases for individuals with chronic illnesses, co-morbidities, and disabilities, serving as the single point of contact.
- Document short and long-term service and support goals in collaboration with the member's care team.
- Identify members that would benefit from alternative levels of service or other waiver programs.
- Mentor and train new staff, assisting in formal training and process improvement initiatives.
- Submit utilization/authorization requests to utilization management with supporting documentation.
- Report critical incidents to appropriate internal and external parties, including state and county agencies.
- Assist in appeals, member grievances, and state audits.
Requirements
- BA/BS degree and a minimum of 2 years of experience working with a social work agency, or equivalent education and experience.
